IMPORTANT INFORMATION FOR PATIENTS WHO DON’T HAVE INSURANCE OR WHO ARE NOT USING THEIR INSURANCE
(“SELF-PAY PATIENTS”)

Under a new law, self-pay patients have a right to receive a “Good Faith Estimate” of the total expected cost of non-emergency medical services and items, which may include medical tests, prescription drugs, equipment and hospital or ASC fees.

If you are an eligible self-pay patient, you will receive a Good Faith Estimate after scheduling your item or service, or you can request one before you schedule your item or service.
You should save a copy of your Good Faith Estimate, as you can dispute a bill that is more than $400 higher than your Good Faith Estimate.

Vascular & Surgical Care Northwest is not required to provide a Good Faith Estimate if you are insured, or if the service or item is provided within 3 days of scheduling it. The estimate can change based on new or corrected information regarding your diagnoses, condition and medical needs.

Vascular & Surgical Care Northwest does not provide an estimate of costs for any service or item not provided by Vascular & Surgical Care Northwest or our affiliate Podiatry Associates Northwest. Please contact other facilities, laboratories and other providers if you require a Good Faith Estimate for those items or services, even if ordered by one of our physicians.
